Sorry to respond so late !
<txt> and <concept> under those elements are in fact legal.
I had been wondering about them too, did a little digging, and found out
they had in fact been added at some point in time.
Please consult changes history at the beginning of DTD. You will find a
note about how and when these were added.

For DDI 2.1 the cannonical version is the DTD. I will ask the guys
handling this on the SRG to look into the problem and get back to the
list. (they may be doing it already :)
