[DDI-users] CATI - type surveys and the DDI

Currently this is handled by the <forward> element in <var><qstn>. Note
that this is repeatable so that you can indicate the qstn ID in the
attribute qstn and provide the logic in the PCDATA for example:

<forward qstn="Q2">If response eq 1</forward>
<forward qstn="Q4">If response gt 1</forward>

Q2 and Q4 can have similar <backward> information indicating the possible
previous questions.
